1. Scope of Services and Compensation

Home Bridge Collective LLC (“HBC”) provides educational, informational, and advisory services to help families navigate senior care and housing options.

Advisory services are provided at no cost to families. HBC is compensated through referral fees paid by senior living communities, care facilities, or service providers when a placement occurs. You are under no obligation to choose a community recommended by HBC.

2. Professional Boundaries

HBC does not provide medical diagnoses, clinical assessments, treatment plans, medication management, or hands-on healthcare services. Families remain responsible for consulting qualified healthcare, legal, and financial professionals before making final decisions about health conditions, clinical safety, Medicaid eligibility, powers of attorney, estate documents, insurance, or financial commitments.

3. Family Decision-Making and Third-Party Resources

HBC may suggest communities, care facilities, or outside professionals for convenience and exploration, but does not guarantee the accuracy, completeness, or availability of information or services provided by third parties.

Final decisions about community selection, medical care, legal documents, financial commitments, and other professional services remain the responsibility of the family and the appropriate licensed professionals they choose to consult.

4. Privacy Disclosure

HBC may collect contact information and high-level details about your family's timeline, preferences, and care needs to respond and help you explore appropriate options.

HBC treats your information with care and limits sharing to the minimum details reasonably needed to help identify communities or service providers that may be a fit. Please do not submit medical records, Social Security numbers, insurance numbers, or other highly sensitive information through the website form. If more detailed information is needed later, HBC will discuss the best next step with you directly.

5. Emergency Situations

HBC's services are consultative only and are not equipped to handle medical emergencies. If there is an emergency, acute illness, or sudden change in physical or mental condition, contact 911 or the appropriate healthcare provider immediately.

6. Phone Consultations

If a family calls before completing the website form, HBC will provide a verbal disclosure at the start of the conversation and follow up by email with this full disclosure for the family's records.
